COVID-19 Patient Instructions

Dear Valued Patient,
We are pleased to announce that Boulder Eye Surgeons will re-open on 4/28/2020! We have implemented some new protocols aimed at keeping all of us safe.


Please do not come to your appointment if you have a fever, a cough, or are experiencing shortness of breath or flu-like symptoms.

All Clinic Appointments

Prior to entering the clinic:

  1. When you arrive, please call our office from your vehicle, at 303.444.3000, and select option 1 to check-in.
  2. Please wait in your vehicle until we contact you to let you know that we are ready to see you. We will then direct you to the appropriate entrance. Entrance 1 is at the front of our office and Entrance 2 is on the west side of our office, across from Walgreens. Both entrances will be marked.
  3. Please arrive wearing a mask/face cover.
  4. Upon entering our office, your temperature will be taken and you will be asked some questions regarding your potential COVID-19 risk factors.
  5. You will be directed to clean your hands.
  6. We ask that you do not bring someone with you to your appointment, unless absolutely necessary. If you do bring someone with you, that person will need to follow these protocols as well. NO MORE THAN ONE EXTRA PERSON PER PATIENT.
  7. You will be taken to a room immediately. If there are not any available rooms, you will be asked to wait in the lobby/hall at least 6 feet away from other people.
  8. All optical patients will be by appointment only.

As you can imagine, rescheduling clinic appointments is a bit daunting, so we appreciate your patience. Some of you have been contacted already to reschedule your visit. We are working diligently to contact all patients. If your appointment has not yet been rescheduled and your need is urgent, please contact our office at 303.444.3000 or [email protected].


Due to changes in practice protocols, previously scheduled appointments starting May 4th through the end of the year may have to be rescheduled. Please contact our office at 303.444.3000 or [email protected] to check the status of your future appointment.

All Surgical Patients

Please be patient with us as we navigate through getting you rescheduled. You will be receiving a phone call within a few weeks from one of our surgical schedulers to get you scheduled back in for your procedure at our new, state-of-the-art, surgery center that is dedicated only to eye surgery.

Additional Precautions and Information

  • All staff will be wearing masks and goggles, and washing/sanitizing hands after each patient interaction.
  • We will be transitioning over to a virtual check-in very soon, but until that time all pens, clipboards, etc. will be wiped down after each patient.
  • All exam rooms will be thoroughly cleaned after each patient.
  • All lobby surfaces, doorknobs, etc. will be cleaned multiple times throughout the day.

How to schedule an appointment

Please call us at (303) 444-3000 to make an appointment with Drs. Keller, Nichols or Cuevas. Don’t forget to ask for your personalized web portal username and login to the portal below and fill in your medical history information as well as your preferred pharmacy and primary care/referring provider before your appointment.

Personalized Web Portal

Boulder Eye Surgeons has partnered with Modernizing Medicine to provide electronic medical records and online patient access to their medical information. Through the portal, you can access your electronic chart as well as educational material and is an easy and confidential way to communicate with our office! If you do not have a username and password or have forgotten your password, call us at (303) 444-3000 to gain access.

What to expect as a new patient

It is hard to predict how long your first visit will last, but new patient visits typically last from 1 to 2 hours, depending on the eye problem. Subsequent visits are usually substantially faster. We appreciate your time and a top priority at Boulder Eye Surgeons is to minimize the time our patients spend waiting. You can further reduce the amount of time you spend in our office by logging into the web portal and submitting your medical history as well as your preferred pharmacy and primary care/referring provider. In addition, please download and complete the Intake Form and HIPAA acknowledgment in the Patient Forms section below and bring it with you to your appointment.

Patient Confidentiality and HIPAA

The Health Insurance Portability and Accountability Act (HIPAA) was passed by Congress in 1996. Its primary purpose is to improve the efficiency and effectiveness of the healthcare system, help providers access patients’ health care information, standardize the way the information is handled, and ensure that patient health information remains strictly confidential.

No issue is more sensitive to people than keeping their personal health information private. Patient confidentiality is a top priority for Boulder Eye Surgeons. The HIPAA law was enacted to provide safeguards for personal health information from disclosure, fraud, and abuse. Health care providers are required by law to notify all patients of the healthcare provider’s privacy practices. By law, no health care can be provided until you acknowledge the receipt of our privacy practices. You can download a copy of our privacy practices in the Patient Forms section below. We apologize for the inconvenience of this additional paperwork in an already complicated health care system. More details about this federal law can be found on the government’s HIPAA website.
