Boulder Eye Surgeons has partnered with Modernizing Medicine to provide electronic medical records and online patient access to their medical information. Through the portal, you can access your electronic chart as well as educational material and is an easy and confidential way to communicate with our office! If you do not have a username and password or have forgotten your password, call us at (303) 444-3000 to gain access.

What to expect as a new patient

It is hard to predict how long your first visit will last, but new patient visits typically last from 1 to 2 hours, depending on the eye problem. Subsequent visits are usually substantially faster. We appreciate your time and a top priority at Boulder Eye Surgeons is to minimize the time our patients spend waiting. You can further reduce the amount of time you spend in our office by logging into the web portal and submitting your medical history as well as your preferred pharmacy and primary care/referring provider. In addition, please download and complete the Intake Form and HIPAA acknowledgment in the Patient Forms section below and bring it with you to your appointment.

What to expect as a new patient

The Health Insurance Portability and Accountability Act (HIPAA) was passed by Congress in 1996. Its primary purpose is to improve the efficiency and effectiveness of the healthcare system, help providers access patients’ health care information, standardize the way the information is handled, and ensure that patient health information remains strictly confidential.

No issue is more sensitive to people than keeping their personal health information private. Patient confidentiality is a top priority for Boulder Eye Surgeons. The HIPAA law was enacted to provide safeguards for personal health information from disclosure, fraud, and abuse. Health care providers are required by law to notify all patients of the healthcare provider’s privacy practices. By law, no health care can be provided until you acknowledge the receipt of our privacy practices. You can download a copy of our privacy practices in the Patient Forms section below.
